Complete and print the form.
Documents needed for a lost passport application:
Affidavit from a Solicitor explaining the circumstances of loss
Original Birth Certificate
Proof of I.D. (proof must include your photo i.e. Driving Licence, Social Services card, Age card, etc)
Proof of Address (utility bill, bank statement, etc)
Proof of Occupation (certificate or letter from employer)
Police Report (or PULSE number)
Four passport size photographs
Completed application form:
- Guarantors must have signed and dated it – No. 11 on completed form
- Declaration by Applicant must be signed and dated - No. 12 on form
- Witness must have signed, dated and stamped – No. 15 on form
A Postal Order payable to the Ghana Consulate Dublin for €140.00
A postal stamp for €8 (new passport will be returned by registered post)

Your application will not be processed and will be returned to you if you do not include all of the above documents.
When your application has been accepted your name then goes on a list and you will be called to come in person to our office in Dublin for your appointment with the Biometric team from the Ghana High Commission. At this appointment you will have your photo, fingerprints and signature captured for the issuance of your passport a few weeks thereafter.
